Tip is lined up with graphics down (0) however since I play little plus on my Ping Drivers, the grip is aligned so the graphics are a little to the right (right handed golfer). I attached a picture to show what it looks like from address - or you could replace the grip and set it back at neutral (0).
Extension doesn't harm/hurt the playability but needs to be disclosed as this wasn't to me. I believe that if you put a Callaway or TaylorMade tip on this, it would play 45.5 or 45 if you removed the extension.
